The CD-ROM (stock no. 024-004-02430-5) is available from the U.S.
Superintendent of Documents, Box 371954, Pittsburgh, Pa. 15250-7954. The cost to customers outside the U.S. is US$17.50, and US$14 for U.S. residents.
